Avoid starting two copies of MPE

I am using Windows 7 and MPE 1.8.6.
MPE seems to have a mechanism to avoid starting a second instance. If I have MPE minimised to tray then start another instance, normally the existing instance is opened instead.
However sometimes I seem to end up with a second instance. This is generally quite confusing, because the second one fails to connect to the phone.
Unfortunately I can't figure out why it sometimes fails, or any scenario to cause the bug to happen reliably. In case it makes any difference. I often use a hotkey-based launcher app to start the MPE process. Even with this launcher, it normally works fine - so it might not be anything to do with the launcher.
Can anyone suggest anything? E.g. diags I could collect, or something I might be doing wrong, or any ideas at all? Perhaps a quick look at the source code for existing instance detection might give a clue how it could go wrong???
